Sixline wrasse problem

It started out a day ago looking like a small scratch on the left side of his head (not really sure). It has since gotten worse. It looks swollen with scales starting to pop up. Its also starting to swim funny, i attached a video

Ive had it less than a week, i have it in a 2 1/2 gallon quarantine tank. I had my water tested by the lfs yesterday and everything tested good.

Sorry i cant get a good picture with my dirty glass and the fish constantly running away from the camera.


Any idea whats wrong?

My lfs recomended a vitamin garlic food mixture which i started today (i'd been just feeding pellets twice a day), Plus melafix antibacterial treatment.

The dreaded ich? I wasnt thinking ich because it never seemed to scratch on anything.

From what i read the stress of a QT should make it clearly visible (white dot wise) and i didnt see anything other than the messed up side of its head.

Also is sleeping in a mucous ball normal for a sixline? because it did that the first night. i read conflicting reports on whether that's normal.

I think it looked more like an injury, I didn't see anything that looked like ich to me. Maybe Juan sees something I don't?

My 6 line leaves a "mucus ball", I see it the next day in my birdsnest coral where he sleeps.
