Is it the Mandarin or something on the rock or coral. Either way give as much of a description as you can if you can't get a good pic.
Also give us the age, size and parameters of the system. Also anything that has changed recently.

I think in that pic there is a mandarin facing the cam and its got a white spot up to the left of its face by its eye

Lymphocitus? Looking at the Kole Tang anyhow is my guess. If so nothing to do but maintain healthy water and feed good.
