Need Ballast

My ballast fried after 5yrs plus of service.

I dont know where to get a replacement. I am in no way an electricial, so all the jibberish you see below is what I could actually read on the sticker.

If you could please direct me to the correct company, I would appreciate it.

Here is the information that came off the ballast

One the side stamped: 11011129

On the Sticker:

Model 200

Listed Lamp Balast 34EF

2x55W BIAX LAMPS OR 2XF40 T-12

120V HIGH POWER FACTOR
INPUT 120VAC 5-/6-HZ

MAX CURRENT 1.5AMP

I believe an IceCap 430 will work for you. I would contact them and ask to be sure. I can not tell what the manufacturer of your ballast is.

The manufacture of hte ballast (OMG i cant believe I forgot to put that on the origianal post)
