Removed one of the heaters; cleaned the overflow boxes; removed the Maxi-Jet 900's and replaced with the Hydor Koralia #2's
Moved the overflow more toward the middle of the tank. The corner location where it was below was more restricted on one side since it was up against the glass. This location should work much better. I also removed the Penguin 350. I was mainly using it for mechanical filtration, but now everything has cleared up with all the die-off of concern. I went through a dozen of the filter pads, but it was well worth it.

Here is a current full tank shot. The lawnmower blenny has created himself a little hill of sand in the middle/front part of the substrate. I guess he didn't like it totally flat.
