HELP! Is my anenome dying?

Hey guys.

Some info, I have 20 Litre Nano nano tank and it has been going well for just over 2 months.

A week ago i bought a bubble tip anenome and it was doing okay for the first few days but in the last two days I think it has really been struggling, it squished himself under a rock and late last night i found him almost at the top of the tank on the glass.
he is currently on the sand bed with his tentacles looking awful, he is still attached to a bit of rock though.

he used to be double the size also.

So is he dying?
How can I fix this and make him healthy?
