Not too sure if this is cotton mouth or another type infection/disease.
Water quality is fine
0 ammonia
0 nitrites
10 nitrate

I have to agree with @Uncle99. This seems to be cauliflower disease, also known as Lymphocystis. You can do a search on the disease, look at some of the picts and see if you agree. There isn't too much you can do about it.

However, it is possible that it is something else, especially if the clown got damaged in the mouth area.

Thanks guys. I just read up on that and does appear to likely be what I am seeing.
The water parameters are and have been stable since the cycle finished nearly two months ago. I have also done a minimum of 20 and maximum of 30 gallon water changes weekly(180 ga system)
Can’t believe it could be water quality at this point but I might send off a sample to be fully tested and see what the results are.

Thanks again
